Member Forms

All members are required to fill out and submit these forms each year.  Forms include: Member Information, Volunteer Form, Member Agreement, and Member Authorization.

Financial Aid Form
The financial aid form is optional and intended only for those individuals applying for financial aid.  Due to many great supporters and donors, we are able to offer financial aid for tuition, after the deposit has been applied.  Financial aid is first come, first serve, so please fill out the form below and return it to the NCAS office.

Member Handbook

The NCAS Handbook outlines policies and procedures for the Nebraska Choral Arts Society.  The membership agreement that each singer and parent signs states that you will fully abide by all policies and procedures set forth in the NCAS handbook.  Please call the office with any questions regarding the handbook.
